Adverse Event Reporting
1) What is ADR ?
Noxious and unintended response to a medicinal product if a medicine properly prescribed (normal dose, route etc.) and administered.
ए.डी.र. क्या है?
यदि किसी दवा को ठीक से निर्धारित (सामान्य खुराक, मार्ग आदि) और प्रशासित किया जाता है, तो एक औषधीय उत्पाद के लिए हानिकारक और अनपेक्षित प्रतिक्रिया।
2) What to Report ?
a) All types of suspected adverse reactions like known or unknown, serious or non-serious and frequent or rare.
b) Reactions from pharmaceutical products.
क्या रिपोर्ट करें?
ए) सभी प्रकार की संदिग्ध प्रतिकूल प्रतिक्रियाएं जैसे ज्ञात या अज्ञात, गंभीर या गैर-गंभीर
और अक्सर या दुर्लभ।
ख) दवा उत्पादों से प्रतिक्रियाएं।
3) How to Report ADR?
Use the “Suspected Adverse Drug Reaction Reporting Form/Medicine side effect Reporting Form which are available on the official website of IPC (www.ipc.gov.in) to report any ADR

Filled ADR form submitted to the nearest ADR Monitoring Centres (AMCs) or directly to the NCC-PvPI.
A reporter can also email the suspected ADR form at

ए.डी.र. की रिपोर्ट कैसे करें ?
किसी भी ए.डी.र. की रिपोर्ट करने के लिए आई.पी.सी. की आधिकारिक वेबसाइट (www.ipc.gov.in) पर उपलब्ध ” संदिग्ध प्रतिकूल दवा रिएक्शन रिपोर्टिंग फॉर्म” या “मेडिसिन साइड इफ़ेक्ट रिपोर्टिंग फॉर्म” का उपयोग करे।
पूर्ण रूप से भरा हुआ फॉर्म अपने नजदीकी ए.डी.र. निगरानी केंद्र (AMC’s) पर जमा कर सकते है या फिर सीधे NCC-PvPI पर जमा कर सकते हैं।
4) Who can Report ?
All healthcare professionals (Clinicians, Dentist, Pharmacist, Nurses, Physician, Physiotherapist etc) and all non-healthcare professionals including consumers/patients can report ADRs.
A reporter can report ADR via helpline number i.e.
1800-180-3024 (Monday to Friday, 9:00 AM to 5:30 PM)
ए.डी.र. कौन रिपोर्ट कर सकता है ?
सभी तरह के हैल्थकेयर प्रोफेशनल्स जैसे की क्लिनिस्ट, डेंटिस्ट, फार्मासिस्ट, नर्स, फिजिशियन, फ़िज़ियोथेरेपिस्ट आदि या फिर गैर हैल्थकेयर प्रोफेशनल्स जिनमे उपभोगता और और मरीज शामिल हैं। यह सब ए.डी.र. की रिपोर्ट कर सकते हैं ।
ए.डी.र को नीचे दी गयी हेल्पलाइन नंबर के माध्यम से रिपोर्ट कर सकते हैं:-
१८००-१८०-३०२४ (सोमवार से शुक्रवार, सुबह ९:०० से शाम ५:०० बजे तक)
